Woodlands Student Ministries offers a number of events throughout the year; from simple one-night activities to week-long missions trips. Here you will find permission slips/participation waivers for upcoming events that require documentation.
Medical Release Form/Parental Consent Form - All students & friends of students who travel with us MUST have these forms on file with us. If your insurance plan changes our office needs to be notified. This form will stay on file with us through your students Senior Year of high school. The medical form travels with us to all events off campus. It's imperative that we have CURRENT medical/contact/insurance information on file in case of a medical emergency.
